All customers are urged to bet within your limits.  If you suspect you have a gambling problem, you may request a voluntary New York Statewide self-exclusion from all gaming entities licensed, permitted or registered by the New York State Gaming Commission.  You must complete the form below, and if mailing, have the form notarized and attach a photo ID.  You can also obtain the form at any OTB office.

You may also request to place a limit on the amount of daily or weekly wagering in your wagering account.  Please contact customer service for more information on self exclusion and voluntary limits on wagering accounts.
